Application to Participate
TheraMatrix Physical Therapy Network(TPTN) currently administers the outpatient physical therapy benefit to eligible members for the following groups:
- UAW-Ford Hourly Actives
- FCA US – UAW and Other Bargaining Unit Members
TPTN members are required to seek outpatient physical therapy services at an in-network provider.
There are no fees or dues to participate in our network, however, there is a one-time processing fee of $100. Our Provider Relations staff is available to assist our in-network providers Monday through Friday during normal business hours.
If you are interested in joining our network or would like to add a location to an existing contract, please complete the form below and click submit. A member of our Provider Relations department will contact you with more information about the network and our contracting process.
Change or Terminate an Existing Location from our Network
